Healthcare compliance training runs $14 to $90 per employee per year, and the spread is almost entirely about who wrote the courses. Here are the published rate cards, three real headcounts priced out, and the vendor that rules itself out of healthcare on its own site.

Healthcare compliance training software costs roughly $14 to $90 per employee per year for the platform and content combined, and the spread is almost entirely about who wrote the courses. General-purpose learning platforms that you load your own content into run $25 to $45 per person per year. Compliance-content vendors that ship ready-made HIPAA, OSHA and harassment courses run $14 to $90 depending on how many topics you license and how long you commit. The enterprise names most hospital systems shortlist, Cornerstone, Docebo, Absorb and SAP Litmos, publish no rates at all.
That is the honest answer, and it is not very useful on its own. So the rest of this page works through what each published vendor actually charges, what a 60-bed clinic and a 2,000-employee health system realistically pay, the one vendor that rules itself out of healthcare on its own website, and the three costs that never appear in a quote.
What healthcare compliance training software actually has to do
Before pricing anything, it helps to be precise about the job, because healthcare buyers are usually paying for four different things at once and vendors bundle them differently.
First, annual regulatory training. HIPAA Privacy and Security awareness, OSHA bloodborne pathogens, hazard communication, workplace violence prevention, and in most states harassment prevention. OSHA reads "at least annually" as within a period not exceeding 365 days from the last session, so this is a rolling obligation per person, not a calendar event.
Second, credential tracking. Nursing licenses, BLS and ACLS cards, DEA registrations, fit-test dates, competency sign-offs. These expire on their own schedules and the platform has to warn somebody before they do.
Third, the audit record. A dated, per-person export that survives a Joint Commission survey, an OCR inquiry or an OSHA inspection. Retention rules differ sharply: bloodborne pathogens training records must be kept 3 years, while employee exposure records under 29 CFR 1910.1020 must be kept 30 years. Most LMS vendors default to about 2 years of history, which is a real gap.
Fourth, the content itself. This is the line item that moves the price the most, and the one buyers underestimate.

Two things fall out of that table immediately. Content-inclusive vendors and platform-only vendors are not comparable on the headline number, because with TalentLMS or MoodleCloud you still have to buy or build every HIPAA and OSHA course. And the cheapest published rate in the table, MoodleCloud at $2.44 per user per year, is cheap precisely because it includes nothing else and cannot install plugins.
The vendor that rules itself out of healthcare
This one deserves its own heading because it saves people a wasted evaluation. LearnUpon states on its own pricing page that it will not be a fit for organizations requiring HIPAA compliance to process health data inside the LMS, alongside a 100-user floor for its Employees product and no on-premise option. If you are a covered entity that needs protected health information handled within the learning platform, LearnUpon has answered the question for you before the first call.
That is unusually direct and other vendors are vaguer, which is why the business associate agreement question has to be asked explicitly. Most LMS platforms never touch PHI, because training completion data is employment data rather than health data, and in that common case a BAA is not required. But if you plan to store patient case studies, incident details or anything identifiable inside courses or assessments, you need the BAA in writing before you sign, not as a renewal conversation.
What it costs at real headcounts
Numbers per employee are easier to argue about than to budget with. Here are three realistic deployments.
A 60-person outpatient clinic. Platform-only on a published flat rate: about $2,880 a year at $4 per learner per month. Add content, either licensed at roughly $15 to $30 per person or built in-house from your existing policies. Content-inclusive on EasyLlama Core: $3,597 a year. On Traliant Essential at their quoted structure: closer to $840, though that rate is quoted at 500 learners on a 3-year plan, so a 60-person clinic should expect to pay more per head.
A 400-person specialty group. Platform-only runs $19,200 to $33,600 a year. EasyLlama Core is $23,980. Traliant Complete with Platform Plus lands near $13,976 at their published structure. This is the band where quote-only enterprise vendors start appearing on shortlists, and where you should force them to quote against the same headcount and the same topic list.
A 2,000-employee health system. Now you are in enterprise territory and almost nothing is published. Absorb LMS sizes you into a learner band, in this case 1,001 to 5,000, then routes you to sales. Docebo quotes on yearly active users, which for a mandatory annual HIPAA course means effectively your whole headcount. Cornerstone OnDemand and SAP Litmos both publish feature matrices with no rates. Third-party marketplace reporting puts enterprise LMS contracts around $83,000 a year for Absorb-scale deployments and from roughly $25,000 for Docebo, but those are listing-site figures and should be treated as directional only.
How much does HIPAA training cost per employee?
For the training content alone, budget $15 to $60 per employee per year in the US. A single standalone HIPAA awareness course sold per seat typically lands at the low end, around $20 to $25, which matches EasyLlama's published $24.95 per course per learner per year. A full compliance bundle covering HIPAA plus OSHA plus harassment plus code of conduct runs $60 to $90. The platform that delivers and tracks it is a separate line at $25 to $50 per person per year unless it is bundled.
Do you need a separate LMS for healthcare compliance?
Usually not, and paying for a healthcare-branded platform is often paying for course content you could license separately. What healthcare genuinely needs from the platform is narrower than vendors imply: per-person expiry dates with automatic reminders, re-assignment of the specific training that fixes a lapse, retention that can hold records for the longest applicable period rather than a two-year default, and an export that shows a named person, a named course, a date and a version.
Where a healthcare-specific vendor earns its premium is content maintenance. HIPAA guidance, state workplace violence rules and OSHA interpretations move, and somebody has to update the courses. If you have no compliance writer on staff, buying maintained content is cheaper than maintaining it yourself. The three costs that never show up in the quote
Record migration. If you are replacing an existing system, somebody has to move years of completion history. For a compliance-driven buyer that is not optional, and it commonly adds three to six weeks to a rollout. Ask whether it is included or billed as professional services.
The HRIS integration, and then keeping it alive. Automatic enrollment from your HR system is the feature that makes annual training actually happen, because new hires get assigned without anyone remembering. It is also the thing that fails quietly. A feed that stops writing records does not throw an error anybody sees, it just produces a compliance report that looks suspiciously clean, and the usual fix is monitoring the pipeline for freshness and volume anomalies rather than discovering the gap during a survey. Whatever you buy, put a monthly eyes-on check in the calendar.
Metering. Per active learner and per registered user can differ by 30 to 60 percent on the same workforce, and healthcare has an unusually high proportion of per diem, float and seasonal staff. A vendor that meters monthly active users can look cheap on paper and expensive in a flu-season month when everyone logs in. What should a healthcare organization actually budget?
For a US healthcare employer between 50 and 2,000 people, budget $40 to $70 per employee per year all-in for platform plus content, and treat anything under $25 as either content-free or a promotional first-year rate. Then judge the shortlist on the three things that genuinely differ between vendors rather than on the per-seat number: whether renewals are automated or somebody runs a spreadsheet, whether the audit export is dated per person and per course version, and whether the bill is metered or flat.
A platform that is $10 cheaper per seat but needs a coordinator to chase expiring BLS cards by hand is not cheaper, it has just moved the cost into salary where it never appears on a quote.
